We consent to our child's participation in the Blue Devils Youth Football, Inc. (BDYF) football program. In consideration for permitting our child to participate in the activities conducted by BDYF, Inc. we release, waive, discharge, convenant and reliquinish any and all action or cause of action against BDYF, Inc., it's Officers, Promoters, Officials, Staff, Coaches, and/or Volunteers for personal injury, death and/or property damage occurring to our child as a result of engaging or receiving instruction in the activities conducted by BDYF, Inc. or otherwise while for any purpose participating in activities conducted by the youth football league. We further release all Officers, Promoters, Officials Staff, Coaches, and/or Volunteers from any claim whatsoever on account of first aid, treatment, or service rendered to our child during participation in BDYF, Inc. activities. We agree to return all equipment issued, cleaned and in good condition BDYF, Inc. and we accept the responsibility for the replacement cost of any equipment not returned to BDYF, Inc. by the date notified.
